The vast majority of gym-goers are healthier than the vast majority of fat people. There are a minority of gym-goers that take steroids or exercise to an unhealthy degree, and there are a minority of fat people who have healthy vitals. Overall, people who watch what they eat and exercise regularly are healthier than people who don't do those things.
The man in the first image has a lot of visceral fat, which is the real killer, especially in men, who are more prone to heart disease and similar obesity-related health problems than women are.
A lot of short men can get really muscular, it's actually kind of nice. My husband is about the same height as that man and it didn't take him long at all to naturally become very muscular when he was young. That man's body isn't even abnormally muscular, it's just about what any man with decent genetics could accomplish by going to the gym regularly and eating properly. We are just so used to fat blobs who stuff their face all day that we can't recognize someone who looks healthy and normal.
