Some many, many years ago, when I was a teacher with young children, a Mexican Girl and former student (at the school) died from taking Ecstasy. She was a teenager, perhaps 15 years old, when she died from suffering negative reactions to it. Her younger sister was still at the school, and I attended the girl's funeral service.

She probably had laced stuff. Pure MDMA has a very high lethal dose and almost no recorded overdoses.
